For example, do most employers offer a certain amount of money that would go towards a plan of your choosing, or only a single plan with limited benefits that you have to accept? l know this is a question with a lot of , but something in general would be helpful. So if you do not like what your employer offers you... do you completely have to give that up and by a new plan that is more attractive to you privately?

There is no ''most employers''. Especially now.
My employees (it is mine too) health insurance increases 20% in cost EVERY year.
We charge 20% of the cost to the employees & then deduct the 20% from their pay weekly. Right now it just went from around $11 for a single person, to nearly $14.
l have worked at many companies & it varies. Some companies offered a flexible benefit plan, where the company gave me like $100 a month & then l had a choice between 3 or 4 plans & how much l wanted to pay out of pocket.
Other companies paid nothing & if l wanted insurance l had to pay for it all.
Other companies paid for it all (which is now almost nonexistent).
You wo not know what ur company offers until u r hired.

Employers pick the plan. Larger employers, will have several plans, and you can pick which one of them you want. Most of the time, they pay either a percentage of your cost, or a flat dollar towards your cost.
They deduct your share from your paycheck, add it to their share, and they pay the insurance company.

Every company l have ever worked for has given me a choice of insurance carriers to choose from..They pay a portion every paycheck and l pay a portion..the only time l did not have a choice of insurance providers was when l worked for Blue Cross and Blue Shield and that is understandable.
